Formation flying 101. Never cross over the top.

Way to many mid-air's in the last month, and this one could have been easily avoided. Very unfortunate.

Question though, How the hell did he go over the top and take his rudder off with the 207 wing????
Maybe inverted as doing a roll????!!!!

IMHO:

Damage to right aileron of 207 according to photo. My only guess is that the Caravan guy went up and over the 207's tail from left to right, which would explain the Caravan pilot saying don't pull up. Then after passing over the 207 tail, he came diving down behind the left wing of the 207, and the Caravan guy tried to pass the 207 underneath its right wing. He passed under the wing but not far enough below the 207, so as he passed the Caravan's fin hit the 207 aileron from behind, which is consistent with the damage in the photo. The 207 wing "dragging" the faster Caravan's fin rearward probably imparted a downward bend in the Caravan fuselage (or some part of the Caravan tail was dragged backward pushing the elevator control horn or some part of the elevator control system downward), causing the Caravan to go nose down.

But like it has already been said, whatever happened it was really really stupid and un-necessary. If you're going to fly formation, do it in a way where everyone can see what they're doing. And no need to go that close, especially without a pre-flight briefing and a procedure and a plan.

In last year's Flying Wild Alaska, wasn't there a boy friend/girl friend pair of pilots who hooked up and flew formation home, only without the contact?? I hope it wasn't them.

Just my $.02 but it looks to me like the aileron and wing are bent down, that is what made me think with the slope of the vertical on the 208 that he was upside down when he hit it?? If he was, he would continue down and under her right to left.
I guess we will never know but way to bad a deal! Stupid stuff does catch up to a person some times.
Good thoughts to both of there family's and to Her.
I certainly have to be careful about throwing stones as there is always a glass house close by!! Some times way better to be lucky than good!!

No Fing around with the delivery trucks or your fired from my place. Get your own plane or go and rent a 172 from the guy down the road and do it there.

They are both 135 airplanes, it doesn't matter if you have an agreement and a brief and a plan and communication for formation flight, You Ain't supposed to do it with out betting your certificate on it!!
Now both company's will have to completely revamp there training manuals, 2 some ones or more will get there ASS reamed for not training there pilots to not fly formation, there will be a whole bunch of recurrent training. Chief pilot or whoever takes care of the training manual will have many rewrites to get it OK'd, director of OP's has some very bad days ahead! The FAA will be monitoring every pilot they have for awhile.
If the FAA thinks or believes there was a break down in the training or when they go through the training records for the last 3 years and find anything not done correctly then Shit will hit the fan along with maybe civil penalty or maybe some down time??
Some one dies in a company doing there JOB and it can effect the whole dam Company's moral, Call in grief councilors and a lot of other things that nobody has even thought about yet.

Gee lets fly formation on the way home??
